To download any of the following forms, you will need Adobe Acrobat Reader. Download a free version of Adobe Acrobat Reader here and follow the instructions provided at the Adobe site.

Forms are not set up to be completed online, so they will need to be printed, filled out and brought to the shelter, or they may be faxed to us at:

They can also be mailed to us at the address below. However, adoption applications are best brought in or faxed, as the pet of interest may find a home before the form is received at the shelter:

Adoption Screening form in .pdf. This form is required for anyone wanting to adopt an animal. Once you adopt an animal, you will also need to fill out an adoption contract which is only available at the shelter.

Employment Application form in .pdf. This form is required for anyone wanting to apply for a job at the shelter.

Request for Interview and Information This form is designed for students who need to interview a staff person to meet a class requirement. First preference is given to students who are volunteers at the shelter, and all interviews are granted as time permits. Please fax the form to the attention of Candy Barnes, Humane Educator/Volunteer Coordinator at (540)432-9509, or bring it to the shelter. The application will be given to the appropriate staff person, depending on the applicant's needs.

Volunteer Application Complete this application if you are over 16 years of age and wish to work with our pets, or you can just email humane@rhspca.org to register for the required orientation. Cat Surrender Form in .pdf. This form is for individuals who are surrendering a cat to RH - SPCA.
