Adoption Screening form in .pdf. This form is required for anyone wanting to adopt an animal. Once you adopt an animal, you will also need to fill out an adoption contract which is only available at the shelter.

Employment Application form in .pdf. This form is required for anyone wanting to apply for a job at the shelter.

Cat Surrender Form in .pdf. This form is for individuals who are surrendering a cat to RH - SPCA.

Dog Surrender Form in .pdf. This form is for individuals who are surrendering a dog to RH - SPCA.
